Adult Day Health Care (CBAS)

CBAS serves adults 18-100+ with chronic health conditions in a safe and caring day center setting, to support their ability to live in the community and out of institutional care.

We are an interdisciplinary team of nurses, social workers, occupational, physical, and speech therapists, and dieticians integrating health and human services inside and outside of our center walls.
